Associates' Fund: How to Apply

If your program meets our guidelines, please complete the following steps:

 

Step by Step Directions: In a concisely written 2-3 page proposal, please provide a short narrative explaining the following information below. Please be sure to label each section with the appropriate headline. 

 

I. Organizational Overview –Who Are You?

· Mission

· Key Programs

· Who you serve

  

 II. Goals, Outcomes and Impact-What Will You Do?

· Please offer a short summary explaining your proposed use of your grant

· What are proposed measurable results?

 

III. Financial Information-How Much Will It Cost?

· Budget

 

IV. Assessment-How Will We Know You Are Successful?

· How do you define success?

· How will the project be evaluated ?

 

Additional Information Request: Please attach the following supportive materials to your proposal in a PDF format.

 

· Contact Person and Information

· A list of your board of directors

· Total agency budget

· Annual report (if available)

· Evidence of the organization's tax-exempt status

· W9 form

· A photo that shows your organization at work

· Please email your organization's proposal to Lianne Hughes at Lhughes@grandcirclefoundation.org

  

Grant Recipients:

· Grand Circle Foundation grant recipients are required to provide a summary report of the project funded within one year of receiving the grant and updated photos.

 

Deadlines:

· Spring 2013: March 25, 2013

· Fall 2012: October 26, 2012
